Hypertension in the South Side Essay The population of the Southside Neighborhood in Chattanooga, Tennessee is roughly six hundred and seventy three people. The races residing in this community are blacks, Hispanic, native Hawaiian and other races. The black community is the largest followed by the Hispanic community. In the population of this community, the males are three hundred and eighty while the females are two hundred and ninety three. In addition to this, in this community, about two hundred and fifty people have hypertension. The people with high blood pressure are mostly the African American adults and the old people as well as people from the Hispanic background. Literature Review Hypertension or high blood pressure basically means high tension or pressure in the arteries. Arteries are vessels that carry blood from the heart to all the organs and tissues of the body. In addition to this, hypertension does not necessarily mean excessive emotional tension though stress and emotional tension can temporarily increase the blood pressure of an individual. The normal blood pressure of a person is below 120/80 and the blood pressure between 139/89 and 120/80 is known as pre-hypertension and a blood pressure of 140/90 or above is considered high blood pressure (Weir, 2006). The top number is the systolic blood pressure and it corresponds to the pressure in the arteries as the heart contracts and pumps the blood forward into the arteries (Weir, 2006). The bottom number is the diastolic pressure and it normally reflects the lowest pressure to which the arteries are exposed to (Weir, 2006). An elevation of the diastolic or the systolic blood pressure increases the risk of developing cardiac disease, renal disease, hardening of the arteries, and stroke and eye damage. These complications of high blood pressure are normally referred to as end-organ damage due to the fact that damage to these vital organs is the result of chronic hypertension. For this reason, the diagnosis of hypertension is vital so that efforts can be made to normalize blood pressure of a person and thus prevent further complications. According to Turncock (2007), the American Heart Association approximates high blood pressure roughly affects one in three people in the U. S. It is also expected that high blood pressure will affect about two million children and teens and this clearly portrays that high blood pressure is a major health problem in America. Hypertension normally tends to rise with age and this explains the reason that some of the old people in the Southside Neighborhood have high blood pressure. Most of these old people in this neighborhood are older than fifty years old. The most common form of hypertension that they have is the Isolated systolic hypertension. This kind of hypertension only occurs when the systolic blood pressure that is the top number is high. About two adults out of three adults in this community over the age of fifty five who have hypertension have the isolated systolic hypertension (Weir, 2006). In addition to this, the African American population in the community tends to have more severe high blood pressure and is likely to have it earlier in their lives according to the Chattanooga Hamilton Health Department. However, most of them are more likely to be aware that they have hypertension and normally get treatment faster than the Hispanic community residing in the Southside Neighborhood. On the other hand, according to the Chattanooga Hamilton Health Department, the members of the black community have higher rates than the Caucasians of other causes of death other than die from hypertension related complications such as kidney failure and stroke (Turncock, 2006). The Southside neighborhood faces some risks that are prone to hypertension in the future. For instance, a significant number of some teenagers in the neighborhood are obese or overweight and this makes them more likely to develop pre-hypertension. Being overweight is having extra body weight from bone, muscle, water and fat while being obese is having a high amount of fat deposits in the body. However, fewer adult women in the neighborhood have hypertension. In the Southside Neighborhood, there is a community asset that supports the local public health system in promoting health and improving the quality of life. This asset is the Chattanooga Hamilton County Health Department. And it has been quite successful in carrying out its mandate. The Chattanooga Hamilton County Health Department main roles are to track and investigate health problems and hazards in the Southside community. It has been quite instrumental in advocating for healthy eating habits in an attempt to curb the high rates of obesity that are quite apparent in most teenagers in Southside neighborhood. The Chattanooga Hamilton County Health Department has collaborated with the Tennessee Department of Health to use the populating health information systems such as the hypertension registries in an attempt to find out the kind of people at risk of contracting hypertension. In the Southside neighborhood, the residents are somewhat exposed to air pollution and this can have an adverse effect on them that may lead to hypertension. Polluted air has a negative effect on a person due to the fact that blood normally per fuses all of the organs and carry beneficial substances and toxic substances to other organs. In most cases, air pollution is the source of materials that may enter the bloodstream through the mouth, nose, skin as well as the digestive tract. Harmful chemicals such as lead, benzene and heavy metals, volatile nitrites, carbon monoxide, herbicides and pesticides. According to Stoto (2000), these substances are known to produce harmful effects on the bone marrow, blood, lymph nodes and spleen. In addition to this, the blood cells of a person are regularly undergoing turnover with new cells entering the circulation as the mature cells are lost thus making the blood system very vulnerable to environmental poisoning. For instance, lead normally interferes with the normal formation of the red blood cells by inhibiting some significant enzymes. Furthermore, lead damages the membranes of the red blood cells and interferes with the cell metabolism is a manner that somewhat shortens the survival of each cell (Swayne, 2006). In addition to this, some airborne chemicals normally stimulate the immune system to activate macrophages and leukocytes that can produce extensive damage to the tissues, especially to the cells that normally line the blood vessels. Therefore, the combined effect of these events is to speed up changes that ultimately lead to hypertension or high blood pressure. Therefore, the health departments in Southside neighborhood, Chattanooga should ensure that the residents do not come across heavy metals such as lead that is commonly found in children toys and they should also carry out epidemiology programs in an attempt to be aware of any diseases that might be detrimental to the community. Additionally, this health department should raise the residents awareness of the chemicals discussed above as they can cause hypertension or high blood pressure. The local public health system, private medical practitioners and other interested parties in the Southside neighborhood should come together and adapt a Less Stress Health Program, which will consist of seven parts. These parts should be plan well, move well, pray well, eat well, sleep well, think well and abstain well. If adapted by this neighborhood, this program can prevent hypertension as well as the other diseases that are normally borne out of high blood pressure such as renal failure. By enjoying life well, moving well and eating well, the residents of the neighborhood can reduce the chances of contracting high blood pressure. In addition to this, this program should help the residents of the neighborhood to lead healthy lifestyles by taking measures such as developing fitness plans, lowering their blood cholesterol, lowering body weigh, improving their sleep, relaxing through spiritual nourishment as well as developing hopeful and positive thinking patterns and support (Rowitz, 2009). In due time, all these factors if taken seriously by the residents of the Southside neighborhood, then hypertension and other diseases can be an issue of the past. Additionally, this program should adapt the four steps to better health. The first step will be to record the health data. Secondly, with the aid of any health educator is to set goals in each area of the neighborhood. The third step should be to start to make some needed changes and create less-stress health seminars that will offer advice to the neighborhood on how best to combat or prevent hypertension. The fourth step would be to repeat the health record after every three and seven weeks in order to efficiently serve the neighborhood with the necessary advice regarding how to live or prevent hypertension (Weir, 2006). Furthermore, this program should hold campaign against unhealthy behaviors such as lack of physical activity, poor diet and tobacco use as they are risk factors for getting high blood pressure. In addition to this, obesity and overweight are risk factors for hypertension and this program needs to cater to the young people in the Southside neighborhood as some of them are already obese and this can lead to hypertension or other serious illnesses. Lately in the Southside neighborhood, private and public programs are being designed to promote healthy behavior among the youth. In addition to this, employers are becoming more aware that obesity and being overweight as well as smoking as these habits are affecting the productivity and health of their employees and in the end, this affects the businesses. The result of this is that innovative employers are providing their workers with a range of work-site –based health promotion and programs that prevent diseases such as hypertension. In the long run, these programs will improve the health of the workers as well as increase their productivity and capitulate a noteworthy return on investment for the employers (Pinger, 2008).

Skin is undoubtedly crucial in the maintenance of the bodys internal balance also protecting externally. It gets severely compromised in burns, non-healing ulcers, reconstructive surgeries etc. bringing down the patients quality of life drastically. Tissue engineering is a more efficient approach than traditional skin grafting. It is a research area that is forever evolving, where researchers are always working towards one united goal, i.e. to develop in bulk quantities, a skin substitute that can be handled with less care, integrates faster with the bodys natural matrix and costs reasonable. INTRODUCTION: A lot of research has been undertaken in the past to realize that it is possible to generate skin by applying engineering techniques. This is done by growing skin at a faster rate than normal and in an artificial manner4. Skin is the largest most widely spread organ in the human body. Its role is to protect the bodys internal environment from harshness of the external conditions and restrict entry of microbes, by acting as a barrier4. Several situations diseases arise, due to which the skin gets irreparably damaged, thus requiring interventional help in restoring it back to health. Tissue engineering is the application of engineering techniques to develop biological substitutes1. Burns (acute) ulcers (chronic) are the most common conditions which require the replacement of skin. In developing countries (Fig 1)2,3, due to lack of knowledge on safety, a very high number of burns accidents occur every year, and mostly of fatal nature. The fatality is mainly due to pain, infection, loss of body fluids incapability of the body to self-regenerate large amounts of lost skin5. Thus, experimentation in this field was triggered. Many skin diseases, which lead to necrosis, pigmentation problems, also require engineered skin4. CLINICAL NEED FOR SKIN TISSUE ENGINEERING: In most incidents, both the epidermal and dermal layers require replacement. Conventionally, the treatment method involved skin grafting i.e. autologous (self) split-thickness and full-thickness grafting, where healthy skin was taken from other areas of the body and replaced at the injury site. Split-thickness (comprising of the epidermis a part of the dermis) grafting is not a logical method to use, when a large area of the body (>50%) is affected less healthy skin is available. But, it is todays gold standard approach6. Further trauma, due to grafting, can be painful to a patient who is already in a critical state. Also, scar formation post-operatively is another reason for its reduced usage. Full-thickness grafts are suitable to use when the burned area is less than 2% of the total area. These problems could be avoided if skin (of full or partial thickness) were grown artificially and substituted in the place of real skin6. LITERATURE REVIEW: The skin can be broadly divided into two layers i.e. the epidermis dermis. The epidermis is made up of several layers and may/may not consist of extra-cellular matrix (ECM). The layers from surface to deep are: cornified, granular, spinosal and basalar layers. The most commonly found cells here are the keratinocytes melanocytes. The dermis is constituted by GAGs proteins. Within the dermis, fibroblasts are most commonly found8. Several skin substitutes exist for wound coverage in tissue engineering4. It can be broadly divided into temporary and permanent skin substitutes. The table below (Table 1) is a list of all the material options available for skin replacement: Table 1: Temporary and Permanent Skin Substitutes8 Permanent tissue engineering of the skin can be broadly divided into three categories6, 8: Epidermal replacements Generally, using autologous keratinocyte sheets. Replaces only the epidermis, but â€Å"take rates† are very poor, suitable for superficial burn treatment only. Dermal replacements Replaces only the dermal layer. In most cases, it is applied along with an epidermal graft to improve â€Å"take rates†. Dermo-epidermal (bilayer skin) replacements Replaces both the epidermis and dermis. Suitable for full-thickness burns. Skin replacements have two main components i.e. cells and the scaffold. In wound coverage, three types of cells can be used autologous, allogenic or stem cells. Autologous (self) cell usage is the most preferred as it is easily accepted by the patients body does not need incite and anti-immune responses. Allogenic (donor) cells, if used directly can lead to the eventual rejection of the transplant. However, it is used in an acellular fashion, where the donor keratinocytes are removed prior to culturing9. Stem cells have trans-germal pluripotential properties are currently being researched for their poteintial application in skin engineering. Less information is obtained on keratinocyte stem cells. The suggested reason for their longevity is that KSC cycles very slowly and is resistant to mutations8. The type of biodegradable scaffold, either natural or synthetic permits cells to attach onto them and facilitate handling during transplantation6, 9. Rheinwald Green Experiment8: The experiment carried out in 1975 by Rheinwald and Green where human (autologous) keratinocytes were produced in-vitro, proved to be a breakthrough in this field and modified versions of this method are used nowadays. Extracted keratinocytes were allowed to form colonies on a plastic substrate. These colonies expanded to form a sheet. Stratifications arose as the daughter cells, usually at the centre, started multiplying vertically and a 12-cell layer was achieved. To increase the multiplicative capacity of keratinocytes, a feeder layer (comprising murine Swiss 3T3 lethally irradiated fibroblasts) mitogens were introduced to the culture. Epidermal Replacements: A small skin biopsy of the patient is harvested, which is cultured to produce a patch. The full-thickness biopsy of the patients skin is cut finely and enzymes are added to cause disaggregation of the skin into cells. A feeder layer, as mentioned previously, is used to culture these cells in culture flasks. To promote proliferation, epidermal growth factors, enzymes such as insulin, hydrocortisone, cholera-toxin and bovine serum are used. After colonies have been formed, trypsin is added. The KCs are cultured to confluence and later, the sheets are removed from the flasks (using dispase) for use8. The result of this method compared to the split-thickness gold standard is quite poor, as the dermal layer is missing and it depends upon the health of the dermis existing. Also, it is prone to scarring, takes too long, expensive, extremely fragile and has varying â€Å"take† rates6. Dermal Replacements: It was claimed, in 1952, that using only pure epidermal sheets, success would be lesser than compared to those with a dermis10. To accentuate the success of the epidermal transplantation, dermal replacements were constructed. A dermal replacement that covered the affected area with cryoprserved allogenic skin was used minus the epidermal layer was used11, 12. Also, an observation that allogenic keratinocytes elicited more anti-immune response than allogenic fibroblasts, was reported. To reconstruct the dermis, the two-stage Integra application is most widely used now13. This dermis functions as a scaffold for the attachment of keratinocytes and improves vascularization9. Burke et al (1981) developed a dermal replacement, where a collagen sponge was covered with a silastic layer (synthetic). The sponge behaves as a scaffold for the fibroblast cells. This technique was commercialized into a product (Integra Dermal Regeneration Template) 9, 14. A modification to this employed GAGs along with collagen, in the scaffold. Here, a precipitated mixture of bovine collagen fibres and a chondrotin-6-sulfate (GAG from shark cartilage) was freeze dried. This generated a collagen-GAG sponge scaffold, which had a mean pore size. Cross-linkage to strengthen the matrix was done using gluteraldehyde. Finally, the silastic layer was applied. This is available as a product; Integra Artificial Skin (Chamberlain and Yannas, 1999)9, 15. According to Heimbach et al (1988), this is most suitable for burns patients. The concept of using absorbable polymer scaffolds (synthetic) such as polyglactin 910 or polyglycolic acid was the next improvement in dermal replacements. Here, allogenic fibroblasts are enzymatically cultured and this culture is mounted on the polymer scaffold for integration). Due to this, an ECM consisting of collagen, growth factors, GAGs etc. is formed, which stays active even after it is frozen17. This was commercialized as Dermagraft 8, 16. Two-stage dermis application has shown proven results, and now clinical trials are being conducted to examine the applicability of one-stage dermis, such as Matriderm 6. The dermal replacements essentially require an epidermal covering. Dermo-Epidermal Replacements: These are available both as autologous or off-the-shelf products. In autologous DED replacements, both keratinocytes and fibroblasts are harvested from the patient and are added to the collagen-GAG scaffold. Cultivation of this in culture medium is for around four weeks. This is a more permanent solution 6, 18, 19. The first model of todays Apligraf was done by Bell et al (1979)20. DEDs use human keratinocytes fibroblast cells (allogenic) within a scaffold. Morphological studies after using Apligraf reported the presence of a well-defined epidermis, with all four layers, as in the natural skin, and seeded allogenic fibroblasts aligned in a normal manner within the collagen matrix 8, 21. COMMERCIALLY AVAILABLE PRODUCTS: CONCLUSION FUTURE AIMS: Tissue engineering of the skin was the first to be approved by the FDA has evolved a great deal, from the first application of only cultured keratinocytes to the use of biological skin substitutes. Research is still in-progress to develop skin in bulk quantities, mainly for burns patients, and to mimic all the mechanical and properties and functions of the natural skin. The state of the art results can be achieved now by using cultured keratinocyte cells with the dermal replacement, Integra, in full-thickness, small and clean wounds. This has shown optimal results in cosmesis and wound closure8. However, this branch of tissue engineering is still very much in a developing level. Studies to analyse how to reduce various risks in patients, who receive donor cells should be done. Also, a main difficulty is in getting the cells to attach to the dermis, post-transplantation. Burns patients are highly susceptible to various problems, thus there is a need for materials that present lower risk than animal/human materials. Mainly, it is ideal if the graft starts to behave like natural skin soon after grafting, which is possible only with rapid vascularization and cell implantation. The Creative Classroom :: Education Teaching Teachers Essays

The Creative Classroom When I was three years old, my parents and I made the most significant move of our lives– we moved from northern Ohio to my mother’s hometown in southern West Virginia. Many things about my life changed because of our relocation, but most significantly was my mom’s job. While in Ohio, she had stayed home with me, but once we came to West Virginia, she resumed her career as a teacher. Immediately, I was thrown into the school system. I remember being so proud that my photograph had been included in the 1987 Man Junior High School Yearbook– and I was just four. Since then, I have viewed the teaching profession through my mother’s eyes and consequently, the eyes of a teacher. I have seen the joy that comes with teaching as well as the frustrations, and I look forward to them all in my future as an educator. I consider my views of teaching to be realistic. I have seen firsthand the paperwork, confrontations with students, extra hours required, and the stress that comes with teaching. But, I have also seen a teacher’s entire day be turned around just because one student grasped a new concept. I have seen a student’s eyes sparkle when they discover something new. And beyond that, I have seen a teacher’s eyes sparkle when his or her pupil rejoiced in their newfound knowledge. I welcome all these experiences into my life, and cannot wait to make my own discoveries in my secondary mathematics classroom. Because of my observations of the education field and my own personal style, I feel that my classroom will best be served with a diverse group of philosophies including essentialism, perennialism, behaviorism, and social reconstructionism. Essentialism is the "back to basics" approach to education that concentrates on teaching primary skills to students. Essentialism tugs at the very heart of mathematics: in order to understand the more complex theorems and algorithms, you must first have a firm grasp of the basic principles of mathematics. I believe learning mathematics is both a step-by-step and conceptual process. With essentialism, I hope to highlight the step-by-step approach. In this, I will be constantly reviewing elementary mathematical concepts, building to a better understanding of the given concept and further building up to a more complex mathematical understanding. Perennialism emulates the belief that ideas that have lasted over centuries should be the concentration of the classroom.

A Dolls House: The Analysis of Nora and Her Case of Leaving Her Family

In Henrik Ibesen's play A Doll House, Nora Helmer struggles with telling her husband, Torvald Helmer, the truth about a loan she receives for them to go to Italy when he was sick. Consequently, when Torvald learns of the news he instantly insults Nora and declares that she has "ruined [his] happiness" (Ibesen 93). However, when Torvald tries to dismiss his insults after receiving a note that her contract was revoked, she does not accept his apologizes and decides to leave Torvald and her children to "make sense of [her]self and everything around [her]" (Ibesen 100). Her selfish decision to leave makes her a bad wife and mother, but she there are a few more characteristics that makes her a bad wife. The characteristics that Nora shows in the story are her dishonest, her individualism, and the unfortunate failings of the father figures during in her life. In the story, Nora is in a troubling situation because she borrows money from Krogstad, Torvald's employee at the bank, to pay for a trip to Italy so that Torvald could recover from an illness and is now being blackmailed by Krogstad because Torvald wants to fire him. Not only does borrow the loan, which something her husband is fully against, she lies to Torvald saying that she receives the money from her father. The borrowing of the money without her husband's approval, and her lie saying that it is from her father are two dishonest acts on Nora's part, acts that a wife should not do. In a review produced by Fabienne Oguer, he calls this situation a "Trust Game" (Oguer 85). Both characters, Torvald and Nora, hypothetically play a game in which one may has to trust the other. In the beginning of the story, Torvald is forced with the decision to either trust Nora and marry he... ...terrible decision to leave Torvald, trapped within his doll house. While most commonly thought of social problems are based on discrimination and stereotypes, one specific social problem that does not fit into these standards is global warming. Global warming is â€Å"an increase in earth’s average atmospheric temperature that causes corresponding changes in climate†¦ that may result from the greenhouse effect† (Dictionary.com). As a social problem, global warming is a fairly large and broad topic. The greenhouse effect is an environmental condition caused by excessive quantities of carbon dioxide, water vapor, methane, and nitrous oxide in the atmosphere. These definitions are simply physical definitions. In terms of being a social problem, global warming and the greenhouse affect yields harmful effects on every community and society as a whole. This type of social problem does not choose its victim based on race, gender, sexual orientation, class, income, age, or any stereotypical category a person can be placed in. Not every person is equally affected by this, but everyone is susceptible to dealing with this problem. The greenhouse effect is a factor in causing global warming. Global warming  is caused by many factors and different kinds of pollution. The main type of pollution that affects global warming is air pollution. Air pollution is simply defined as pollution of the atmosphere, which ties in with the greenhouse effect. Air pollution can also be defined as â€Å"the addition of harmful chemicals to the atmosphere. Pollution can be considered as an â€Å"undesirable state of the natural environment being contaminated with harmful substances as a consequence of human activities† (Dictionary.com). People may not realize this, but their actions directly affect their surroundings. Completing daily tasks such as driving a car has detrimental effects on Earth’s atmosphere and pollution levels. Global warming causes physical harm to humans, animals, and plant life. A change in the Earth’s climate could be detrimental to the Earth’s life. The physical harm that is inflicted on all of living life due to global warming poses as a problem: a social problem. Global warming does not choose its victim based on any type of criteria such as race, age, gender, sexual orientation, class, income, or any stereotypical category. Global warming poses as a social problem by affecting a significant number of people, and by threatening the stability of the community’s environment. Since pollution is one of the main factors in causing global warming, it also comes with health effects. These effects range from being short-term to being long-term. Short-term effects can be as simple as headaches nausea, and allergic reactions such as irritation to the eyes, nose, and throat. Infections such as bronchitis and pneumonia can also occur. An example in history of health effects from pollution can be found in the â€Å"Smog Disaster† in 1952 located in London. Research has shown that younger children and elderly people are more sensitive to pollution compared to the general public. â€Å"Continual exposure to air pollution affects the lungs of growing children and may aggravate or complicate medical conditions in the elderly† (Lbl.gov). â€Å"Young children and elderly people often suffer more from the effects of air pollution† (LbL.gov). With global warming not being in full affect, the extents of the effects are currently unknown. The effects of global warming are only just beginning with mild winters and extremely hot summers. The climate change can be subtle or dramatic depending on how quickly it occurs. The increase in temperature in the climate either way will be harmful. With temperature increase’s ice caps in the Artic and Antarctica are melting at increasing rates. The melting of ice caps can have many changes includes the destruction of the habitat in the Artic as well as rising sea levels. â€Å"Global warming could push sea levels about 40 percent higher than current models predict† (NationalGeographic.com). â€Å"Models suggest that by 2100 sea level[s] will be between four and thirty-five inches†¦ higher than it was in 1990† (NationalGeographic.com). Ocean front properties and beaches can be destroyed and submerged in water because of these rising sea levels. The higher the temperatures and climates increase, the faster the sea levels will rise. A comparison of this at a minor scale could be like a house built in a flood plain next to a river. The more it rains the higher the water will rise. If the water rises to high, it will flood the flood plain, and destroy the house. Coastal areas will be affected similarly. A person could think of the coastal areas as a flood plain for the oceans. Ice caps melt from the rising temperatures, as the water heats up (even just slightly), the water will begin to expand. As the water expands the sea level will rise. Solutions to global warming and its effects range vastly. There are several different types of solutions ranging from micro-level solutions to macro-level solutions. Solutions can range from fuel-efficient vehicles to more environmental laws. Solutions to global warming may not fully diminish the problem, it may only reduce it. Renewable resources could play a main role in the solutions to reducing global warming. A key point is that if we were to switch to renewable resources it would cost the government a lot of money, which could result in higher taxes or some other means of obtaining the necessary amount of money. A plus to using renewable resources is the fact that in the long run they would pay for themselves. Not only would it provide jobs for the people who would be forced to leave the nonrenewable resources, it would also be opening up opportunities for the unemployed. Along with making our environment a safer and more hazard free living zone, we’d also be leading towards a better economy. Our current economy state is negatively thought of, and switching to nonrenewable resources could put our government in debt. After so many years renewable resources will not only repay its own debt, but it will also produce a profit. If we were to invest in new solutions for global warming it would be a great cost. If we were to pay for the projects to reduce global warming that we believe will have an effect may put our country in great debt. If the country were to switch to fuel-efficient and renewable resources in all institutes and factories it would be a great cost.
